  1. Release and Waiver of Liability: Make sure to read and fully understand the Release and Waiver of Liability before proceeding with the registration.

  2. Required Fields: All sections marked as "required" must be filled out. Incomplete forms will not be processed.

  3. Volunteering is FREE, but your donation is welcome. To send your contribution, please follow the below:

    Important: Ensure that the name displayed is "Zamboanga del Norte Association of Illinois."

Release and Waiver of Liability 

By electronically signing this Release and Waiver of Liability, I, the Hudyaka sa Chicago 2025 Volunteers, acknowledge and agree to the terms set forth in this document for participation as volunteer in the 2025 HUDYAKA sa Chicago Event Tournaments (the “Tournament”) from May 17 to August 10, 2025, at various Hudyaka event locations (the “Premises”).

I understand that by participating in the Tournament, I am voluntarily assuming all associated risks, including but not limited to the risk of injury, permanent disability, or death. I further acknowledge that these risks may arise from the actions of others, the rules of play, the condition of the Premises, or equipment used during the event, and I understand that there may be risks that are not known or not reasonably foreseeable at this time.

By submitting this electronic form, I confirm that I have read and understood the terms of this waiver, including the assumption of personal responsibility for any injury, damage, or loss arising from participation in the Tournament. I agree to release, waive, and discharge the Zamboanga del Norte Association of Illinois (ZAMNAI) and its affiliates from any liability for injuries, damages, or losses incurred, and agree to indemnify and hold harmless ZAMNAI from any legal actions or claims related to our participation.

This electronic acknowledgement serves as the equivalent of a written signature and is binding in the same manner.
